YOFLOW™
LIABILITY WAIVER AND RELEASE FORM

1. I understand that it is my responsibility to consult with a physician prior to my participation in any activity. I represent and warrant that I am physically and mentally fit and have no medical condition which would prevent me from full participation in any exercise or activity related to “YOFLOW”. I acknowledge that it is my responsibility to inform my doctor immediately, and discontinue all moving meditation activities, if I incur any injury because of doing YOFLOW.
2. By my participation in any activity facilitated by Sabrina Universal Lawton, or her organizations, Evolve To Love & Love To Evolve, herein referred to as “YOFLOW”, I agree to assume full responsibility for any risks injuries or damages known or unknown that I may incur.
3. I recognize that activities related to “Yo-flow” including, but not limited to dances, stretches, workshops, private retreats, my own personal residence, online use of the YOFLOWtv APP, trips to the beach or any public or; including Sabrina Universal Lawton’s place of designation, or other any other destination where my involvement in moving meditations may cause injury; I understand that I responsible for any and all risks associated with engaging in such activities.
4. During live YOFLOW events in which I may voluntarily participate; I do acknowledge there is no obligation for any person to provide medical care during, prior to or after any activity related to “YOFLOW”, I do hereby give permission for staff to provide first aid, administer prescribed over-the-counter medication as prescribed or directed by participant and/or aid in seeking emergency medical attention if needed. In the event medical treatment is provide it to me I hereby waive any claim against Sabrina Universal Lawton, Evolve To Love, Love To Evolve, also DBA, “YOFLOW”; nor their employees’ subcontractors or agents for any injury damages or even death caused by any provisions of such medical care or lack there-of.
5. I, my heirs, employer, and/or representatives release, waive, discharge and covenant not to sue for any claims against Sabrina Universal Lawton, her organizations, their employees’, subcontractors or agents for any injury, damages or death as a result of participating in “YOFLOW” activities of any kind, both on and off private locations or public premises.
6. I understand the YOFLOW is a subsidiary of non-profit organization Love To Evolve. Due to the low cost and high value of my membership, enrollment, or subscription; I understand that refunds are not permissible. I understand that I may cancel my membership at any time by emailing [email protected]
7. I have carefully read this waiver and release. I understand that if I disagree with the terms of this release, I have a right to forfeit my participation. I understand that by enrolling in this offering, I voluntarily consent and agree to the above terms and hereby release Sabrina Universal Lawton, Evolve To Love, Love To Evolve, DBA “YOFLOW”, their officers, employees, subcontractors or agents from any damages, loss, injury, or liability whatsoever suffered as a result of my participation in all aforementioned activities.
8. I chose to believe that God will fulfill His promises to me which includes optimal health and wellness by engaging in any and all YOFLOW moving meditations and activities.
